Diagnostic of pathogens causing nosocomial diarrhoea

Streszczenie: Diagnostyka biegunek szpitalnych jest bardzo istotna ze względu na różnorodność czynników je wywołujących, zwłaszcza zakaźnych, takich jak: Salmonella sp., Clostridium difficile, Campylobacter sp., rota-, adeno-, norowirusy czy Cryptosporidium sp. Ze względu na szeroki profil patogenów podczas diagnostyki kału biegunkowego wykorzystuje się wiele zróżnicowanych metod, np.: posiew, badania immunologiczne poszukujące koproantygenów i/lub toksyn bakteryjnych, mikroskopię czy badania genetyczne. Wszystko to wiąże się z różnymi sposobami pobrania, przechowywania i transportu materiału do badań. Należy zwrócić szczególną uwagę na procedury dotyczące wymienionych etapów przedlaboratoryjnego postępowania z próbką, opisane i dostarczone przez laboratorium współpracujące z oddziałem szpitalnym. Czynności te źle wykonane mogą skutkować odrzuceniem próbki i niewykonaniem oznaczenia. Końcowym etapem badania mikrobiologicznego jest przygotowanie wyniku. Powinien on zawierać informacje o poszukiwanych drobnoustrojach oraz w przypadku metod wykonywanych testami komercyjnymi: nazwę zestawu, jego producenta, czułość i specyficzność. Polskie dane epidemiologiczne wskazują na duży odsetek udziału Clostridium difficile w zakażeniach szpitalnych, co zdaniem Autora jest ściśle związane z częstszym zlecaniem badań w tym kierunku, w stosunku do ilości zleceń na badania wykrywające inne ważne klinicznie drobnoustroje (wirusy, Campylobacter sp).

Abstract: The process of diagnosing hospital-acquired diarrhoea is extremely important due to a wide variety of etiological factors, especially infectious agents such as: Salmonella spp., Clostridium difficile, Campylobacter spp., rota-, adeno-, noroviruses or Cryptosporidium spp. Due to a wide variety of pathogens, many different methods such as bacteria culture, immunological tests specific for coproantigens and/or bacterial toxins, microscopy or genetic tests are used. All this involves various ways of collecting, storing and transporting the sample material for testing. Special attention should be given to pre-laboratory procedures regarding handling of the sample materials, which are labeled and shipped by the cooperating hospital ward. Poor compliance with these procedures may cause specimen rejection and in result no test will be performed. The final stage of the microbiological examination is the preparation of the result. It should contain information about the microorganisms which were sought for and if commercial tests were used, also the name of the kit, its manufacturer, sensitivity and specificity. Polish epidemiological data indicate that a high percentage of all nosocomial infections is caused by
Clostridium difficile. This, according to the author, is closely related to the fact that tests for Clostridium difficile are more commonly commissioned compared to other tests that detect microorganisms (viruses, Campylobacter spp.).
