Presence faecal lactoferrin in patients with Clostridium difficile infection – own observations

Streszczenie: Wstęp Ze względu na lawinowo rosnącą liczbę przypadków choroby związanej z Clostridium difficile (CZCD ) o ciężkim przebiegu, niejednokrotnie prowadzącej w wyniku powikłań do śmierci pacjenta, trwa poszukiwanie prostych testów mogących wspomóc proces diagnostyczno-prognostyczny, a co za tym idzie – szybkie wdrożenie prawidłowego leczenia. Wydaje się, że jednym z takich badań może być badanie potwierdzające obecność podwyższonego poziomu laktoferyny w kale. Celem pracy była ocena przydatności oznaczania podwyższonego poziomu laktoferyny w kale w diagnostyce biegunki związanej z C. difficile u dorosłych. Materiał i metody Badanie przeprowadzono u 18 chorych z potwierdzoną infekcją C. difficile (CDI), leczonych w Oddziale Chorób Zakaźnych Centrum Medycznego w Łańcucie w latach 2011– 2012. W oparciu o klasyfikację Zara, na podstawie ciężkości stanu klinicznego ocenionego przy przyjęciu do szpitala, chorzy zostali podzieleni na dwie grupy: o lekkim (I) i ciężkim (II) przebiegu CDI. U wszystkich pacjentów w pierwszej dobie hospitalizacji wykonano test potwierdzający obecność podwyższonego poziomu laktoferyny w kale. Wyniki U chorych z biegunką związaną z C. difficile, niezależnie od stopnia ciężkości choroby, stwierdzono obecność podwyższonego poziomu laktoferyny w kale oraz białka CRP we krwi. W grupie II poziom CRP był znacznie wyższy niż w grupie I. Różnica średnich wartości stężenia białka CRP między badanymi grupami była istotna statystycznie. Wnioski Potwierdzenie jakościowe obecności laktoferyny w kale u pacjentów z CZCD – pomimo iż nie jest przydatne do oceny ciężkości stanu klinicznego chorego – bez wątpienia potwierdza obecność stanu zapalnego w obrębie błony śluzowej jelit.

Abstract: Introduction Due to rapidly increasing number of cases of Clostridium difficile-associated disease (CDAD ) and its severe course, often leading to complications that result in death of the patient, the search for simple diagnostic tests is being conducted. These tests should support the diagnosis and prognosis process and as follows enable promptly introduction of the proper treatment. It seems that the confirmation of the presence of elevated lactoferrin in the patient’s stool may be one of such methods. The aim of this study was to assess, if the detection of elevated fecal lactoferrin (FL) in the diagnostics of C. difficile-associated diarrhea that occurs in adults, is useful. Material and methods The study was carried out at the Department of Infectious Diseases in Lancut Medical Centre during the period between 2011– 2012. It concerned 18 patients with the acknowledged C. difficile infection (CDI). All patients were divided into two groups depending on the course of their disease: mild (I) or severe (II), based on Zar’s classification of the severity of clinical condition that was performed during admission to the hospital. In the first day of hospitalization, the test to confirm the presence of elevated lactoferrin in stool was performed. Results The presence of elevated fecal lactoferrin was confirmed in all patients with C. difficile-associated diarrhea regardless of disease severity. The increased level of CRP protein was acknowledged in all patients. The CRP level was significantly higher in the group of patients with the severe course of disease, than in the group with the mild course of CDAD. Conclusions Although qualitative confirmation of lactoferrin’s presence in stool of patients suffering from the C. difficile-associated disease is not useful to assess the severity of the clinical condition of the patient, no doubt acknowledges the presence of inflammation of intestine mucous membrane.
